Pharmacist Per Diem Rotating Position Summary
Provides consultative services to medical nursing and pharmacy staff. Provides prescribed medications, drugs and other pharmaceuticals and compounds medications. Interprets physician medication orders for appropriateness. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties.
Responsibilities
Processes physician drug orders in a manner that evaluates them for appropriateness (safety and efficacy) for the specific patient (evaluation factors include the drug, dose and interval, indication, allergies, drug-drug or drug-food interactions, patient age, patient weight, & renal function).Processes orders accurately, including computer entry and review of the technician fill before the drug leaves the Pharmacy. Follows through with necessary action in processing orders that are not consistent with practice standards or policies and procedures. Records physician/nurse interactions taken with regard to physician medication orders. Carefully checks the work of supportive personnel; bag checks, cart fill checks, drug repackaging checks, IV’s, etc. processes and prepares sterile drug orders with proper order and care, using aseptic technique. Monitors physician medication orders for consistency and appropriateness with patient age. understands that there are dosage variations between newborn, pediatric, adult, and geriatric patients and knows where to find appropriate information. Answers questions and provides information to physicians, nurses, patients, and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Dispenses controlled substances only after proper documentation and record keeping requirements are fulfilled. Completes monthly perpetual inventory checks as assigned. Completes area inspections as assigned in the time frame assigned. Corrects noted deficiencies found during inspections. Instructs students and other medical personnel on matters pertaining to Pharmacy. Provides on-call coverage as assigned as outlined in Department Policy.
Assesses and prioritizes a rapidly changing workload. Completes work in a timely manner. Maintains a clean and orderly work environment. Shares responsibility for ordering and stocking medication and keeping the department clean. Utilizes personal protective equipment to protect from exposure to chemical, radiation, and infectious hazards.
Participates in department management by providing input at meetings and reading communication. Mentors new and less experienced staff and students. Adjusts work schedule to departmental needs. Uses policy and procedure manuals and other reference material to ensure proper course of action.
Identifies, proposes solutions, and assists in implementation of ideas for improvement in patient care and departmental / hospital operations. Participates in the successful implementation of change. Identifies opportunities and implements strategies to decrease waste in medication. Participates in adverse drug reaction reporting and drug utilization review.
SPECIAL SKILLS: Must be able to work in a fast-paced environment with occasional periods of above average pressure. Must have basic computer skills. Excellent customer service skills. Must be willing to work in all areas of the pharmacy services as assigned. Effective interpersonal skills and ability to work with team members required. SUPERVISES: May Supervise Pharmaceutical Technicians in the absence of a director.
THE PHARMACIST PER DIEM ROTATING CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE
MINIMUM EDUCATION: Graduate of Accredited School of Pharmacy.
PREFERRED EXPERIENCE: Prefer one(1) year of acute care experience but will consider new graduate.
REQUIRED CERTIFICATIONS/LICENSURE: Current Pharmacist license in the State of Arizona.
