Position Summary

The pharmacist is a part of the healthcare delivery team and is responsible for the use of medications throughout the hospital. 

Minimum Qualifications

Graduate of an accredited School of PharmacyMaintains current pharmacist license in OregonPossess computer skills for order entry, word processing, clinical interactionExperience with drug use evaluations/monitoring, clinical interventionsEffective communication skills with allied health care practitionersExperience in sterile products preparation, including TPN, ChemotherapyBLS and ACLS within three months of hire and maintain as current 

Essential Job Functions

Modifies medication dosages as determined by age and weight, renal functionCorrectly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errorsMaintains accountability within controlled drug inventoryPerforms aseptic preparation of sterile productsParticipates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
