Positions in this function consult with customers, members, patients, providers, internal departments and/or external third parties on any or all of the following: Consult with and coordinate on going care and treatment plans for members with clinical pharmacists and other providers.

Primary Responsibilities:
Consistently exhibits behavior and communication skills that demonstrate Optum’s commitment to superior customer service, including quality, care and concern with each and every internal and external customer Documents pertinent patient information in the electronic medical record Assists with data management and recording results for month-end reports Assists in Pharmacy Services wellness programs (i.e. transition of care medication reconciliation) by: Retrieving pertinent patient information for new patient referrals or recent hospital/SNF discharge Contacting patients post hospitalization to perform medication review and information gathering Performing patient follow-up procedures Answering incoming phone calls and directing patient messages Coordinating with other healthcare providers, healthcare professionals, and healthcare team members Updates job knowledge by participating in educational opportunities and reading professional publications Participates in process improvement initiatives, staff meetings and departmental meetings Uses, protects, and discloses Optum patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards Adheres to Universal Precautions and safety policies and procedures at all times Other duties as assignedWhat are the reasons to consider working for UnitedHealth Group? Required Qualifications:
High School Diploma / GED (or higher) Possess and maintain an active pharmacy technician license in the state of Oregon 1+ years of experience as a pharmacy technician 1+ years of experience in customer service 1+ years of experience with medical and pharmacy terminology 1+ years of experience with medication therapeutic categories 1+ years of experience with identifying generic name for brand name drugs and vice versa 1+ years of experience with an understanding of indication for medication Basic level of proficiency in Windows and MS applications (Word, Excel)Preferred Qualifications:
National Pharmacy Technician Certification Possess and maintain an active pharmacy technician license in the state of Washington 3+ years of experience working as a pharmacy technician 1+ years of experience in hospital, ambulatory care or other healthcare setting Experience in managed care setting Bilingual (Spanish/English)*All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
