As part of our team, you will use the latest technology and most recent research to achieve the best possible outcomes. **A caregiver in this position works days, 8:00am-5:00pm.** A caregiver who excels in this role will: + Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in service responsibility in a routine, continuous manner, considering the age and clinical condition of each patient. + Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals. + Support clinical pharmacists in providing pharmaceutical care to patients by direct interaction, and formal educational programs. + Maintain expertise in areas of practice through continual learning, journal review and other educational activities. + Educate clinical pharmacists, technicians, pharmacy students, post graduate trainees, allied health professionals, nurses, and physicians in pharmacotherapy within in the specialty area. Minimum qualifications for the ideal future caregiver include: + PharmD or bachelor's degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training + Specialty residency (is required, if hired for a specialty area where specialty residency training is available) + Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire + Current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire (For individuals required to administer immunizations) + Current Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA). For caregivers required to respond to adult or pediatric cardiopulmonary arrest, this is required within one year of hire + Completion of a post-graduate year 2 residency (PGY2) or fellowship + OR completion of a post-graduate year 1 residency (PGY-1) and 2 years of experience as a clinical specialist or an equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area + OR 4 years of experience as a clinical specialist or an equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area Preferred qualifications for the ideal future caregiver include: + Board certification + Obstetric and Maternal Health experience Our caregivers continue to create the best outcomes for our patients across each of our facilities.
