This Clinical Pharmacy Specialist position will provide focused support for high-risk patients enrolled in Medicare Accountable Care Organization (ACO) and Medicare Advantage (MA) value-based contracts. The pharmacist will co-manage chronic diseases, including heart failure, COPD, CKD, and diabetes, for ACO patients attributed to Quality Alliance Independent primary care providers. In addition, the pharmacist will assist with skilled nursing facility transitions of care medication reconciliation and medication access for such patients. The pharmacist will also triage MA patients identified as non-adherent to diabetes medications and assist with MA statin Star measures. All duties will be performed remotely via telehealth.  

 

A caregiver in this position works 8:00am—5:00pm. 

 

A caregiver who excels in this role will:  

Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in service responsibility in a routine, continuous manner, considering the age and clinical condition of each patient. 

Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals. 

Support clinical pharmacists in providing pharmaceutical care to patients by direct interaction, and formal educational programs. 

Maintain expertise in areas of practice through continual learning, journal review and other educational activities. 

Educate clinical pharmacists, technicians, pharmacy students, post graduate trainees, allied health professionals, nurses, and physicians in pharmacotherapy within in the specialty area. 

 

Minimum qualifications for the ideal future caregiver include:  

PharmD or bachelor’s degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training 

Specialty residency (is required, if hired for a specialty area where specialty residency training is available) 

Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire   

Current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire (For individuals required to administer immunizations) 

Current Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA). For caregivers required to respond to adult or pediatric cardiopulmonary arrest, this is required within one year of hire  

Completion of a post-graduate year 2 residency (PGY2) or fellowship  

OR completion of a post-graduate year 1 residency (PGY-1) and 2 years of experience as a clinical specialist or an equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area  

OR 4 years of experience as a clinical specialist or an equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area 

 

Preferred qualifications for the ideal future caregiver include:  

Board certification 

Obstetric and Maternal Health experience

Physical Requirements:

Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. 

Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.

Position often includes reaching, stretching, stooping, climbing steps and sitting.

Must have normal or corrected vision.

Must have ability to clearly communicate verbally by phone or in person
