Are you ready to make a real impact in patient care? We’re looking for a dynamic Meds-to-Beds Pharmacy Extern to join our vibrant Outpatient Transitions of Care team! If you’re passionate about helping patients transition smoothly from hospital to home, this is the opportunity for you!
**What You’ll Do**
+ **Support Emergency Department Requests and Hospital Discharges:** Be a vital part of our team by assisting with requests and ensuring seamless hospital discharges on weekends and holidays.
+ **Deliver Medications:** Bring essential medications directly to patients discharging from the hospital, ensuring they have everything they need for a smooth recovery.
+ **Collaborate with Experts:** Work alongside our highly skilled nurses, providers, and case managers to provide top-notch patient care and support.
+ **Engage with Patients:** Perform technician duties while also consulting with patients, offering guidance and support as they navigate their medication regimens.

**Qualifications**
+ Enrollment in or recent graduation from an accredited college of pharmacy
+ Valid Idaho Pharmacy intern/extern license
+ Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross
