The ambulatory pharmacy navigator program helps HMH Discharge and Specialty Pharmacy patients receive their discharge/specialty medications quickly and efficiently. This allows patients to be discharged from the hospital smoothly and begin their recovery at home or start their specialty medication therapy right away. . The Navigator I plays a crucial role in ensuring patients receive their prescriptions smoothly. They gather all the necessary information, prioritize tasks, address any financial, insurance, or clinical challenges, and make sure the prescriptions are delivered to patients before they leave the hospital or directly to their homes. The Navigator ensures OBRA compliance by ensuring an offer to counsel is made
and that patient medication educational needs are met. The Navigator I functions will include all job functions of an Ambulatory Pharmacy Technician plus additional duties described below.
Responsibilities
A day in the life of a Pharmacy Navigator I at Hackensack Meridian Health includes:
The Navigator duties includes all job functions of an Ambulatory Pharmacy Technician plus additional duties.Establish an initial greeting to patients admitted or who have a clinic appointment and explain services available via the HMH Pharmacies.Performs prescription order entry duties as necessary.Obtains patient authorization to process prescriptions through the ambulatory or specialty pharmacy.Engages in a collaborative process with floor nurses, discharge planners, case managers, social workers, physicians, APN's, PA's, Medical Residents, unit clerks and other teams/initiatives as required to ensure patients scheduled for discharge to home or who have clinic appointments can receive the critical medications needed in a timely manner.Works to determine patients being scheduled for discharge or who have a clinic appointment to ensure HMH Pharmacies are selected by patient choice to provide services.Determines information sources necessary to process prescriptions (e.g., insurance coverage).Identifies and Troubleshoots patient medication access issues related to insurance, financial or insurance coverage and helps to resolve so patients can receive needed medications.Adheres to all established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality.Ensures prescription fulfillment by in-house pharmacy based upon priority needs to allow for timely discharge and/or start of specialty medication.Coordinates and facilitates patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas or home.Ensures patients receive offer for pharmacist counseling and distributes pharmacist contact information in the event patient has prescription-related questions at home.Facilitates distribution of medication-related educational materials and coordinates counseling with staff pharmacist where needed.Assists Ambulatory Pharmacy staff as needed.Performs duties of Ambulatory Pharmacy Technician if required.Ensures all guests/patients receive safe care. Maintain a 'safety first' work ethic. Report any medication errors or medication variances as required by HMH Pharmacy Policy.Other duties and/or projects as assigned.Adheres to HMH Organizational competencies and standards of behavior.Qualifications
Education, Knowledge, Skills and Abilities Required:High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.Excellent written and verbal communication skills.Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.Education, Knowledge, Skills and Abilities Preferred:Retail/Specialty Pharmacy experience.Strong knowledge and experience with PBM Claims and coverage.Licenses and Certifications Required:Pharmacy Technician License.PTCB or Equivalent Certification Required within 180 days of start of employment with the following exceptions:Hired prior to January 2024Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before 1-1-24Contacts:Regular contact with medical personnel and its visitors.
