POSITION SUMMARY
Perform clinical pharmacy duties under the direction of the Lead Clinical Pharmacist (if applicable). Clinical specialists support the development, implementation and maintenance of systems that promote desirable patient outcomes through the use of safe, appropriate cost-effective medication therapies.
ACCOUNTABILITIES
Assists and supports the development, implementation and management of clinical pharmacy services. Provides clinical consultation and clarification to emergency department practitioners; recommends evidence-based medication therapy regimens and monitoring plans; suggests appropriate cost-effective therapeutic alternatives to medical staff as needed. Monitors medication therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory interactions, adverse drug reactions, appropriateness of drug and dose and therapeutic duplications. Provides accurate, adequate and timely drug information to professional staff. Works collaboratively with clinical pharmacy specialists, clinical staff pharmacists, emergency department physicians, advanced practice providers, medical residents, nurses and case management. Attends and participates in the provision of medication therapy services during emergency response scenarios, including but not limited to cardiac arrest, rapid sequence intubation, trauma alerts, and stroke alerts. Facilitates proper information transfer with regard to medication use for patients admitted to the hospital from the emergency department. Provides patient specific medication use teaching for discharge medications when appropriate. Focus on cost avoidance and cost savings due to medication use in the Emergency Department. Supports the development and implementation of treatment guidelines, policies, disease management protocols, formulary reviews, and critical pathways as approved by appropriate committees. Participates in providing didactic and experiential training for pharmacy students and pharmacy residents. Prepares and reports medication use evaluations, adverse drug reactions, and medication error findings to department leadership. Maintains awareness of contemporary trends in emergency medicine through the professional literature and regular attendance at professional organization meetings, institutes and seminars. Supports and participates in the pharmacy’s performance improvement activities including customer service and patient safety initiatives and documents departmental compliance. Performs duties of Staff Pharmacist as needed. Performs other duties as directed.
