Works under the supervision of ambulatory care clinical pharmacists to assist in providing pharmaceutical care as part of an integrated quality system of patient care. Communicates effectively with pharmacists and other health care professionals and works to improve the medication use process. Other responsibilities are carried out as required for the effective, safe, or efficient functioning and operation of the ambulatory pharmacy team. **Your Exceptional skills and Qualifications** + Provide medication therapy management services under pharmacist supervision + Conduct patient phone calls to complete medication reviews or schedule for future visits + Review patients’ medication lists for potential drug-related problems + Communicate drug related problems with supervising pharmacist and clinic staff, when necessary + Participate in performance improvement and population health initiatives + Work with patients and Network resources to overcome medication adherence issues + High School Diploma or GED : The level of knowledge equivalent to that ordinarily acquired through completion of high school. (Required) + Required CertIfications/Licensures: (Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA).
