Essential FunctionsInterviews patients and caregivers to obtain current medication lists and medication historyCommunicates effectively with all demographics to obtain accurate informationContacts pharmacies, long term care facilities, and other locations to obtain patient medication historyDocuments information in the electronic medical recordCommunicates patient information to healthcare providers to ensure accurate medication history list during transitions of careDocuments medication errors or omissions to aid in process improvement
Job RequirementsMinimum EducationHigh school diploma or equivalent PreferredRequired SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Licenses:Registered with the Kentucky Board of Pharmacy in good standing as pharmacy technicianPTCB Certified Pharmacy Technician (CPhT) with in 90 days of hire
