PRIMARY FUNCTION

The Pharmacy Technician will support the implementation and maintenance of medication value-based care initiatives. This role involves coordinating medication management activities, ensuring compliance with value-based care protocols, and assisting with data collection and reporting. The ideal candidate will have strong critical thinking in regards to pharmacology, excellent organizational skills, and a commitment to improving patient care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Support Value-Based Initiatives: Assist in implementing value-based care programs and initiatives designed to improve pediatric health outcomes and reduce costs. Assist in preparing reports on the outcomes of value-based care initiatives for pediatric patients, highlighting successes and areas for improvement. Medication Reconciliation and Management: Assist in verifying and documenting pediatric patients’ medication histories during transitions of care. Support the pharmacist in managing and monitoring pediatric patients' medication regimens in alignment with value-based care protocols. Track and promote adherence to prescribed medications by following up with families and caregivers and addressing barriers. Technology Utilization: Access and update electronic medical records (EMR) to support accurate medication documentation and communication among healthcare providers. Utilize population health management tools to identify at-risk pediatric patients and support targeted interventions. Data Collection and Analysis: Collect and maintain accurate data related to pediatric medication use, outcomes, and adherence for value-based care initiatives. Assist in analyzing medication-related data to identify trends, areas for improvement, and potential cost savings. Patient and Caregiver Support: Provide basic education to caregivers about medication administration, storage, and potential side effects under the supervision of a pharmacist. Communicate with pharmacies and insurance companies to resolve issues related to medication access, such as prior authorizations and formulary restrictions. Collaboration and Coordination: Collaborate with pediatricians, nurses, and other healthcare providers to ensure coordinated and comprehensive care for pediatric patients. Participate in multidisciplinary team meetings to discuss pediatric patients' medication needs and contribute to care planning. Regulatory Compliance: Ensure that all medication management activities comply with regulatory requirements and best practices for pediatric care.

QUALIFICATIONS

EDUCATION:

High school diploma or equivalent; completion of a pharmacy technician training program preferred.

EXPERIENCE:

Minimum of 2 years of experience as a pharmacy technician, preferably in a value-based care or medication management setting. Community pharmacy or insurance company experience is strongly preferred.

LICENSURE / CERTIFICATION

Current Florida pharmacy technician certification (CPhT) preferred, but not required

KNOWLEDGE, SKILLS AND ABILITIES

Strong knowledge of pharmacology, medications, and medical terminology. Proficient in Microsoft Excel Excellent organizational and time management skills. Proficient in using healthcare software and electronic health records (EHR). Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Attention to detail and a commitment to improving patient care. Familiarity with value-based care principles and healthcare regulations.
