Medication History 

Utilize tracking boards and reports from the electronic medical record (EMR) to identify patients to perform medication histories.  Review currently documented external prescription history and contact the patient’s retail pharmacy to resolve any medication concerns.  Update and reconcile the patient’s home medication list in the EMR   Inquire about medication allergies and update the EMR Document interventions made daily in the EMR  Communicate changes in medication history to treatment team Utilize AIDET (Acknowledges Introduces Duration Explanation Thanks) skills while speaking with patients and their families to improve trust, communication, and compliance and improve clinical outcomes Assist in training of pharmacy personnel, when appropriate  Demonstrate excellent communication skills, attention to detail and teamwork
