Pharmacy Technician III - Medical History

As an integral member of the Pharmacy team and under the supervision of a pharmacist, the Medication History Technician is responsible for working with newly or soon to be admitted patients to obtain and document an accurate medication list to help avoid drug duplications, omissions, errors, and facilitate timely Medication Reconciliation by the physician.  The information obtained will be entered into the hospital computerized medication reconciliation application for use by physicians and other healthcare providers to determine medication orders and treatment plans during inpatient and post discharge settings.  Knowledge of retail pharmaceutical terminology, metric systems, computer systems, and attention to detail is necessary, in addition to excellent customer service skills and the ability to effectively work and communicate with patients, staff and outside providers. 

Qualifications: HSD or GED required.  SC Board of Pharmacy Certified Technician required.  Minimum of one (1) year experience in pharmacy required.  Minimum of three (3) years retail pharmacy experience preferred.  Must successfully complete medical history training program within three (3) months of hire date.  Good written and oral communication skills.  Ability to perform effectively within Critical Care/Emergency Department environments.
