Benefits * 401(k) * Flexible schedule * Referral Program * Weekly Pay with Direct Deposit * Joint Commission Accredited Employer Responsibilities (Physical Therapist) * Work cooperatively with physicians to develop care and treatment plan that is appropriate for client’s home environment, especially as it relates to fall risk, interventions, and home safety * Assess for muscle strength, mobility, gait, range of motion and pain and select appropriate treatment and assistive devices, instructing family and caregivers on use of devices * Report client’s response to treatment or changes in condition to the Director of Nursing and physician * Initiate physical therapy program and instructs other personnel, including family members and caregivers, in specific phases of physical therapy with which they may assist with the client care * Prepare clinical and progress summaries, as well as participate in discharge planning. A copy of the summaries will be submitted to the Director of Nursing * Assess client's ability to return to work. * Treat clients, families, and staff with courtesy and respect Responsibilities (Occupational Therapist) * Conducting physical and psychological assessments of clients and developing or following a treatment plan. * Assessing home and work environments of clients and deciding what adjustments are needed. * Advising on adaptive equipment to help clients with daily activities. * Developing physical rehabilitation programs to help clients regain lost skills. * Educating caregivers and family members of clients on patient care and use adaptive equipment. * Evaluating results and progress of occupational therapy on clients. * Maintaining professional knowledge and the technical progress in order to provide clients with the best treatment program available. * Complying with federal, state, and local certification requirements. * Evaluating patient condition regarding physical and basic mental health. Requirements (Physical Therapist) * Graduated with a master’s or higher degree in physical therapy from an accredited physical therapy program * Passed a National Physical Therapy Examination (NPTE) and/or state examination required by the state * State licensure in state where care is being provided * Two (2) years of appropriate experience as a physical therapist. Community/home health is preferred * Adhere to HIPAA and maintain client confidentiality Requirements (Occupational Therapist)   * A bachelor's degree in occupational therapy from a university certified by the ACOTE (Accreditation Council for Occupational Therapy Education) or AOTA (American -Occupational Therapy Association). * Certified by the National Board for Certification. * Compliant with state healthcare regulatory requirements. * Assess client's ability to return to work. * Possess up-to-date knowledge of treatment practices and rehabilitative programs. * Excellent interpersonal communication skills. * Ability to take care of clients with different personalities. * Comfortable using a computer for various tasks. * Ability to assess patient conditions and to work with treatment plans.
