Physical Therapist | Home Health

Join our Home Health team as a Physical Therapist. This is a part-time position, working 3 days a week, 48 hours every two weeks, Monday-Friday 8 a.m.-5 p.m. No nights, weekends, or holidays! 

Job Summary:

This position will give you the flexibility, variety, and autonomy that you deserve as an experienced Physical Therapist.  You will primarily serve the St. Cloud and surrounding areas.  You’ll work with patients of almost all ages and be assigned a dedicated region, giving you the opportunity to truly connect with your physical therapy patients.  You can expect to visit the homes of five or six patients per day and will be able to schedule patients when it’s best for them and for you.  The Home Health Physical Therapist we hire for this role can expect to review treatment orders and medical records, perform evaluations, and establish and implement treatment programs.  Dependent on your experience as a physical therapist, you may be assigned supervisory responsibilities for students, Physical Therapist Assistants, or Nursing Assistant/Home Health Aides.

Qualifications:

Graduate from an accredited school of physical therapy.  One year of experience as a Physical Therapist providing treatment to various patient groups preferred. Licensed by the MN Board of Physical Therapy as a physical therapist or current temporary permit required. License required within 90 days from date of exam. Transportation available for off campus activities required.  A valid Minnesota driver's license, a driving record in good standing, and the minimum amount of insurance liability coverage required. Current American Heart Association (AHA) BLS Healthcare Provider card required prior to completion of unit orientation.

 

To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous CentraCare employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.

Relocation assistance requirements: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from Waite Park and relocate within 30 miles of Waite Park.
