The Physical Therapy role focuses on participating on an interdisciplinary team level to support and educate therapeutic needs of a participant, provide input to participant care plans, and maintain a caseload of participants that can be treated both in the home and in an outpatient gym setting located in our Adult Day Health Center. We are a unique preventative care model compared to the reactive health care systems that most home care agencies adhere to. In Accordance with CHA policies and procedures, federal and state regulations, professional standards of care, and APTA guidelines and code of ethics, the physical therapist evaluates and treats program participants in their home as well as the PACE Center clinic with a focus on quality care, service excellence, cultural competence, and performance improvement.
