We are seeking dedicated Physical Therapists to care for patients in communities across Long Island. This position will cover Baldwin, Freeport, Island Park, Long Beach, Wantaguh and the surrounding areas Job Details Our Home Care Physical Therapists: Review patient charts and medical history Develop personalized treatment plans Teach therapeutic exercises and stretches Use hands-on therapy to promote healing Maintain patient records and adapt treatment plans accordingly Work closely with a supportive patient care team Provide patient and family education for health promotion Provide care to the utmost standard Qualifications: New York State PT license Minimum of one year of recent hands-on experience as a Physical Therapist Valid New York State Driver’s License and reliable transportation. Benefits: 5x 8-hour shifts (8:30AM to 4:30PM) with rotating weekends and holidays Great healthcare benefits Education reimbursement of $7,000 annually Salary Range USD $95,000.00 - USD $120,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
