Physical Therapist - Home Health at Providence St. Mary Medical Center in Walla, Walla, Washington

Schedule: M-F, 8:00 am – 5:00 pm, with weekend rotation to work a Saturday every 4-6 weeks.

The Physical Therapist in the Home Health Program provides for and assesses the physical therapy needs of patients referred for evaluation.  The physical therapist sets up appropriate plans of care under the direction of the physician for the patient.  The Physical Therapist will instruct and support other home health team members in carrying out home programs, including education of patients and caregivers, home health aides, and others who may be working with the patient.  The Physical Therapist will carry out the program as thoroughly and effectively as possible.

Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence.

Required Qualifications:

Bachelor's Degree Physical Therapy Upon hire: Washington Physical Therapist License  Within 30 days of hire: National Provider BLS - American Heart Association This position requires that you obtain an NPI number within 30 days of starting in the position. Preferred Qualifications:

Master's Degree Physical Therapy Or Ph.D. Physical Therapy     1 year Experience in a therapist capacity in a hospital, skilled nursing facility or other related medical field Home Health experience
