ARIZONA GENERAL HOSPITAL **Responsibilities** The Therapist provides patient care including evaluating the patient for treatment providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management while delivering quality patient services. You should be personable, positive, and motivated to provide quality care to a wide variety of patients every day. + Work with a multidisciplinary team of physicians and therapists to develop and execute a treatment plan for each patient + Perform initial patient evaluations as well as re-evaluations to indicate the level of progress + Document all treatments and note any status changes + Tailor treatments to individual patients as needed - don’t be afraid to think creatively or do a little extra + Seek new learning opportunities and be a resource when others need to learn from you + Consistently offer the best care possible to every patient **Qualifications** **Minimum:** Graduate of an accredited Physical Therapy Program required Licensed by AZ State Board of Physical Therapy as a Physical Therapist American Heart Association BLS certification Bachelor's degree **Preferred:** Prior use of electronic health record preferred. Master's or Doctorate degree in appropriate clinical area preferred. 2 years progressive experience in clinical area performing patient clinical care preferred. **Pay Range** $55.95 - $55.95/hour We are an equal opportunity/affirmative action employer.
