Key Responsibilities:

Work collaboratively with Physical Therapists to execute individualized treatment plans. Support and guide patients through exercises, functional tasks, and daily living activities. Conduct follow-up visits for home health patients to assess eligibility for continued services. Provide maintenance therapy and assist patients in reaching their rehabilitation objectives. Regular travel to patient residences is required, necessitating a valid driver’s license.

Qualifications:

Home Health experience preferred but not required

Valid California Physical Therapist Assistant license required

Current CPR certification required

Current California driver’s license, good driving record, and reliable transportation
