Accurate Home Care, LLC is growing in the area of Home Health. We are seeking a  Physical Therapist Assistant to support patient centered care.  This position carries out effective treatment plans for patients.

Pay & Perks:

Essential Job Duties and Responsibilities:

Provide patient centered treatment visits in their individual homes, or within a Senior Living Community Implement client treatment plans with guidance from the physical therapist Instruct patients on physical therapy interventions, modalities and home exercise programs Deliver treatments/interventions for patients that allow for exceptional patient care Understand and comply with policy and procedures set forth by Accurate Home Care Maintain compliance with county, state and federal regulations affecting home health care Must be able to commute within the Twin Cities and surrounding Metro Area A valid Driver's License and reliable vehicle are required Additional tasks, as assigned by manager
