The physical therapy practice involves examination, evaluation, treatment and instruction of individuals for the prevention, habilitation or rehabilitation of physical disability, functional deficit, and or pain. Such practice includes but is not limited to the following areas: acute care, orthopedics, neurology, geriatrics, pediatrics, cardiopulmonary, home health, ob-gyn, OP Rehab and oncology. **Responsibilities:** + Assessing the physical therapy needs of the patient, preparing the rehabilitation plan of care based on the assessment that integrates goals, treatment, outcomes and discharge plan, providing rehabilitation service to the patient as specified in the rehabilitation plan of care, reassessing the patient's response to services provided and revising the rehabilitation plan of care as needed. + Documents care given within the designated time frames in the EMR and hands off problems and resolutions to the case manager and the team. + Incorporates an understanding of economic factors in the delivery of services and maintains compliance with productivity standards of the department as specified in the staffing plan. + Participates in marketing and community education functions as requested. + Plans and implements in-service and continuing educational activities to meet identified learning needs and performance improvement initiatives. Participating in staff education activities and providing consultation to facility personnel. **Credentials:** + Physical Therapist (Required) + Active NJ License by the Board of Physical therapy + Basic Cardiac Life Support (Required) + Current BLS certification from AHA approved course + Driver's License (Required) + NJ Driving License **Education or Equivalent Experience:** + Bachelor of Arts or Science (Required) + Education Specialization: Physical Therapy Equivalent Experience: •And 1+ years Inpatient and Outpatient work experience. + Master of Arts or Science + Education Specialization: Physical Therapy Equivalent Experience: •And 1+ years Inpatient and Outpatient work experience.
