ESSENTIAL JOB FUNCTIONS: Provide physical therapy services as prescribed by the physician that can be safely provided in the home. Conduct initial evaluations, and admission visit within 48-72 hours of referral for patients being admitted for physical therapy; in addition, develop a plan of care.Collaborate with a multidisciplinary team, including physicians, nurses, case managers, OT, to coordinate patient care and optimize treatment outcomes.Provide overall supervision of physical therapy assistants. Deliver evidence-based physical therapy interventions, including exercises, manual techniques, and modalities, to improve patients mobility, strength, and overall functional abilities.Maintain accurate and thorough documentation of patient progress in compliance with regulatory requirements.Educate patients and/or caregivers, families in the care and proper use of equipment and devices such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices. In addition proper exercise, home safety and strategies for preventing injuries to promote long term wellness.Conduct comprehensive assessments of patients conditions and develop personalized treatment plans to address their specific needs.Other duties as assigned
