Qualifications

Minimum Education / Licensure / Certifications 

EDUCATION

Bachelor’s Degree in OT or PT. Master’s Degree in SLP.

EXPERIENCE

Required: None Desired: One year or more. 

LICENSE

Licensed by Florida Department of Health, Division of Medical Quality Assurance. 

CERTIFICATION

Certificate of Clinical Competency (CCC) or documentation that you are in the process of completing CCC, in the case of Clinical Fellowship Year (CFY) SLP.

Position Summary

The Physical Therapist shall demonstrate the clinical ability to evaluate patients’ functional status, plan, implement and document individual therapeutic programs. Performs miscellaneous duties as assigned.
