Schedule: Day Shift, Per-Diem, on-call position

Required Qualifications: 

Bachelor's Degree in Physical Therapy Washington Physical Therapist License upon hire National Provider BLS - American Heart Association upon hire Within 30 days of hire: National Provider Identifier (NPI) number This position requires that you obtain an NPI number within 30 days of starting in the position. Preferred Qualifications:

Master's Degree in Physical Therapy Or Ph.D. in Physical Therapy 1 year of experience in a therapist capacity in a hospital, skilled nursing facility or other related medical field Home Health experience
