Current Service Area: Hampton, Newport News and Suffolk (subject to change as needed to Virginia Beach and surrounding area)

 

Our Home Health Physical Therapists (PT) are the heart of our organization! In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience.

Responsibilities Provide physical therapy services to patients according to a written physician’s plan of careInitiate physical therapy program and instruct other personnel and/or family/caregiver members as appropriatePrepare and submit clinical and progress summariesProvide in-service education programs for nursing organization personnel as neededParticipate in discharge planningSupervise Physical Therapy Assistants according to policy and state regulationsEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in state of operation2 years of experience in adult physical therapyHome health experience preferredCommitment to clinical and documentation excellence About our Line of Business At At-Heart Home Care & Hospice, which achieved a 4.5 quality rating on CMS Home Health Compare, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
