PHYSICAL THERAPIST - St. Joseph Home Care Network- Santa Rosa

Schedule: Full Time, Days

Location: Responsible for West Santa Rosa area

Job Description:

The Physical Therapist assists the physician in evaluating the patient's level of function, develops and updates the plan of care and provides treatment as ordered by the attending physician.

Required qualifications:

Bachelor’s degree from an accredited Physical Therapy Program or Master's degree from an accredited Physical Therapy Program (Based on Graduation Year) or D.P.T from an accredited Physical Therapy program (Based on Graduation Year). California Physical Therapist License upon hire. Within 30 days of hire: National Provider BLS - American Heart Association 

Preferred qualifications:

Home Health experience. 1 year' Experience as a Physical Therapist.
