**Description** PeaceHealth Home Health in Bellingham, WA is seeking a Physical Therapist for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $45.14 – $67.70. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. *Hiring bonus may be available* **Job Summary** Responsible for the evaluation of patients, determining plan-of-care and goals of treatment, participating in treatment of patients within the scope of Physical Therapy. May provide oversight to therapy assistants and/or students in following the plan-of-care and treatment of patients appropriate to the age of the patient served; may provide supervision to therapy aides and/or volunteers. Works as part of a patient care team toward the best outcomes for the patient. **What you will do:** + Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members. + Makes recommendations for relative content and scope of therapy services and implements policies and procedures that guide and support the provisionof services. + Recommends and develops new therapy services and programs in collaboration with staff. + May spend minimal time to assist with evaluating and orienting new caregivers and making recommendations to management regarding equipment and space needs related to physical therapy functions. + Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate. + Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards. + Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource. + May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students. + Recommends and participates in development of new services and programs, quality improvement and monitoring activities. **What you bring:** + Bachelor's, Master's or Doctorat Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted) + Required: Physical Therapist License State of Washington + Preferred: 1 year of Physical Therapist experience + Required Upon Hire: Basic Life Support + Valid driver’s license and proof of auto insurance PeaceHealth is committed to the overall wellbeing of our caregivers: physical, emotional, financial, social, and spiritual.
