The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with plan of care developed for each individual patient and physician’s orders when applicable. Pay Range: $ 90 / per hour - $ 100 / per hour
Essential Duties and Responsibilities:
Performs patient reassessments and regularly reevaluates patient for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriateParticipates in developing the plan of care in collaboration with the physician and other members of the health care teamWhen applicable, completes initial comprehensive assessment of patient and family to determine home care needs; provides a complete physical assessment and history of current and previous illness(es)Assesses and evaluates and/or assists the physician in assessing and evaluating therapeutic, rehabilitative, function status, and home environment (as applicable)Assesses muscle strength, mobility, gait, and Range of Motion (ROM)On incisions resulting from orthopedic surgery, removes staples or sutures and manages incision lineProvides physical therapy treatment and establishes physical therapy plan of careInstructs patient, families, and/or caregivers in the use of and care of therapeutic appliancesDetermines priority needs for physical therapyReports patients’ reaction(s) to treatment or changes in condition to physician and/or Director of Clinical Services or designeeInitiates physical therapy program and instructs, consults, and/or advises other personnel and/or family/caregiver regarding certain phases of physical therapy with which they may work with the patient, and explains the goals of the physical therapy program by participating in case conferencesTrains patient in use of prosthetic device(s) and use of other assistive devices (e.g., walker); ensures equipment used in the course of patient treatment is in good working orderUses appropriate equipment to meet patient goals, cleans and maintains equipment according to manufactures guidelinesPrepares and submits clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in conditionHome setting: Identifies patient and family/caregiver needs for other home health services and refers as necessaryParticipates in discharge planningProvides in-service education programs for other disciplines (e.g., provides in-services to Home Health Aides, Occupational Therapists, etc.) as neededParticipates in peer consultation processSupervises Physical Therapy Assistants according to the company’s policies/procedures and State regulationsParticipates in quarterly medical record reviews as applicableParticipates in care coordination with the company and other personnel caring for the patient to assure that their efforts effectively complement each other and support the objectives outlined in the plan of careProvides/submits accurate and timely documentation in accordance with the plan of care and company policy and proceduresComplies with applicable local, state, and federal laws and regulations regarding professional standards and practice standardsPerforms other duties as assigned/necessaryMinimum Requirements:
Current licensure as a Physical Therapist in the state of practiceGraduate of an accredited school of Physical TherapyOne (1) year of prior professional Physical Therapy experience in an acute care facility, outpatient setting, community health service or home health agency preferred.Must possess and maintain current CPR certification in accordance with State regulation(s)Current record of PPD or chest x-ray resultsAbility to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be requiredComputer proficiency requiredMust be at least 18 years of ageMaxim Benefits:
