Job Details Physical therapist that will provide clinical leadership and oversight as well as ongoing program development for the lymphedema center service line. In addition to providing direct patient care, is responsible for marketing, networking and community outreach for lymphedema center services provided by St. Charles Hospital. Focus is on enhancing and streamlining the clinical services provided to lymphedema patients and building referral relationships. Responsibilities: Maintains the professional standards and clinical competencies of a licensed therapist of same discipline. Fulfills the duties and responsibilities of a licensed therapist as delineated by job description in the role of patient care. Meets all the documentation standards of a licensed therapist as delineated by job description. Maintains ongoing communication with the medical oversight of the program as well as referring physicians in the community. Facilitates orientation of the person served to the services, program and predicted outcomes Uses available financial information in decision making about the provision of services for the person served. Facilitates involvement of the persons served throughout the rehab process Gathers appropriate information and outcomes for the organization to use for performance improvement and follow up Prepares for discharge in a timely manner making appropriate recommendations for equipment, HEP and continued therapy. Includes patient/family in all decisions regarding DC. Prepares and educates patient/family in a prescriptive HEP prior to discharge. Provide patients with necessary resources to continue to manage their condition following DC. Assists with the implementation of discharge/transition recommendations. Coordinate with outside vendors as necessary for garments and supplies necessary for proper patient management. Assists is developing professional/clinical goals for any staff that may be a part of the program. Identifies the training/educational needs for the staff and assures the opportunities to meet these needs are provided Acts as a mentor for new employees participating in the program. Coordinates and provides in-services to staff and community regarding lymphedema management. Participate in performance improvement activities for the outpatient rehab and the lymphedema program. Actively participates in the marketing of the lymphedema rehab program and identifies opportunities to promote the services. Requirements: Education: Minimum Bachelors of Science in Physical Therapy. DPT preferred. Lymphedema certification and/or experience is preferred. St. Charles Hospital is offering to train and pay for the chosen candidate to obtain the necessary lymphedema CDT certification within the first 6 months of employment. Internal applicants included. Licensure/Certification: NYS Physical Therapist license and registration required. Must obtain BLS certification within 60 days from starting. Skills: Physical therapist with strong clinical skills in the area of lymphedema treatment. Must have excellent time management skills and be able to work in a fast paced department. Must have excellent communication and customer service skills and be comfortable interacting with physicians and referrals sources. Must be comfortable with public speaking and providing education to staff and the community. Salary Range USD $47.20 - USD $56.60 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
