Physical Therapist - Outpatient

POSITION SUMMARY:  Responsible for providing a full range of therapeutic services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualification, professional practices, and ethical standards.  A physical therapist (PT) must be able to evaluate a patient’s condition and devise a customized physical rehabilitation and treatment plan. The PTs’ goal is to develop, maintain and restore maximum movement and functional ability, to maximize quality of life in an outpatient rehab setting. The physical therapist shall also demonstrate accountability for the contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion.  The employee may have job functions where they will handle and deliver medications based on the requirements of their job in the assigned work facility.  The employee will complete other duties as assigned.

ESSENTIAL FUNCTIONS:  

Reviews physician's referral (prescription), patient's condition, medical history, and submitted medical records to determine physical therapy treatment required. Plans and prepares written treatment program based on evaluation of patient and incorporate proper interventions that can help patients better perform everyday tasks, decrease pain, ambulation, and improve strength and mobility. Evaluate patients' functional ability through various standardized tests, measures patient's strength, motor development, sensory perception, functional capacity, and level of independence with daily activities, and records findings to develop treatment programs Develop a treatment plan for patients with specific goals and tasks Administers treatments involving application of physical agents, such as cold packs, contrast baths, moist packs, ultraviolet and infrared lamps, Iontophoresis, electrical stimulation, and ultrasound machines Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit Records treatment, responses, and progress in the patient’s electronic medical record and prepare reports as indicated by licensure, the provider, and or insurance recommendations Instructs patient and family in treatment procedures to be continued at home. Completes documentation per clinical / department guidelines and meets expectations of monthly department chart audits.

NON-ESSENTIAL/MARGINAL FUNCTIONS:

Complete required orientation as directed by facility

Follow facility and OSHA safety rules and procedures while on assignment

Follow facility and Joint Commission guidelines and recommendations

Respect cultural and religious practices of patients

Uphold HIPAA regulations

Punctual and dependent for assigned/confirmed shifts

Promoted / educated Tidelands Health Rehabilitation programs to referring physicians, healthcare providers, and or co-workers by performing in-services and/or marketing activities.

Proactively impacts his or her cancel / no show percentage as it relates to improving quality care and patient outcomes.

Achieves the established average monthly patient care productivity metric

REQUIRED/MINIMUM QUALIFICATIONS: 

EDUCATION: Must be a graduate of an accredited therapy program and performed the required clinical internships satisfactorily

 

EXPERIENCE:

 

LICENSURE/CERTIFICATION: Licensed by the state of South Carolina Board of Physical Therapy Examiners

 

KNOWLEDGE/SKILLS/ABILITIES: Ability to utilize therapy equipment in a Tidelands Health rehabilitation setting to provide treatment and procedures preferred.

ADMINISTRATIVE SUPERVISION:  Non required

FUNCTIONAL SUPERVISION:  Functional supervision of physical therapy assistants and rehab technicians

PHYSICAL REQUIREMENTS: Medium While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear. The employee must exert 20 to 35 pounds of force occasionally (activity or condition exists up to 1/3 of the time), and/or 10 to 15 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
