Job Details The outpatient rehabilitation therapist is responsible for evaluation and therapeutic intervention for persons with illnesses or injuries, which impede independent functioning of mobility, activities of daily living, cognitive thinking, and/or vocation. Responds to physician’s treatment orders/ prescriptions in a timely fashion. Performs a thorough patient evaluation and assesses the needs of the patient. Establishes realistic short and long term goals in conjunction with the patient. Goals are measurable. Develops an appropriate and individualized plan of care for each patient and communicates with pt. and family. Demonstrates and applies the knowledge of human growth and development to deliver care in a manner appropriate to the age of the patient. Progresses patients appropriately to achieve the highest potential level of function by discharge. Demonstrates awareness of proper safety considerations throughout treatment sessions and guides supportive staff in the same. Demonstrates awareness of patient precautions / contraindications when delivering care. Demonstrates knowledge and skill in the use of therapeutic techniques, activities and principles. Attends and participates in team/family conferences. Communicates with physicians and other health care providers to coordinate and provide appropriate care. Assesses patient’s status on an ongoing basis and makes modifications to plan and goals as needed The minimum productivity standards that are established are met on a regular basis. Participates in the student affiliation programs under the supervision of the CCCE. Oversees clinical performance of PTA’s, aides, and students to ensure appropriate patient care is being delivered. Initial evaluations are thorough, accurate and completed according to dept. policy within 24 hours. Patient progress is documented on a regular basis with emphasis on patient’s functional abilities. Daily documentation is thorough, accurate and completed according to department policy. Documents patient education activities throughout the admission of the patient. Documentation complies with all regulatory guidelines. Documentation is clear and concise. Makes themselves available to interact/communicate with the persons served, the team, and other stakeholders as appropriate or as requested. Facilitates orientation of the person served to the services, program and predicted outcomes. Communicates as needed with both external and internal resources. Uses available financial information in decision making about the provision of services for the person served. Facilitates involvement of the persons served throughout the rehab process Gathers appropriate information and outcomes for the organization to use for performance improvement and follow up. Prepares for discharge in a timely manner making appropriate recommendations for equipment, HEP and continued therapy. Includes patient/family in all decisions regarding DC. Prepares and educates patient/family in a prescriptive HEP prior to discharge. Assists with the implementation of discharge/transition recommendations. POSITION REQUIREMENTS AND QUALIFICATIONS: Education: Minimum Bachelors of Science in Physical Therapy. DPT preferred. Licensure/Certification: New York State license and registration or valid temporary license. BLS required or will need to obtain a BLS certification by the end of the probationary period. Skills, Knowledge or Abilities: Physical therapist must be able to evaluate and provide treatment interventions to patients with diagnoses ranging from simple orthopedic conditions to more complex neurological conditions. Must be able to work in a fast paced department. Positions requires ability to accurately and completely document in a timely manner utilizing the electronic medical record system EPIC. Must have excellent communication and customer service skills. Experience: Experience in an outpatient setting strongly preferred but not required. Salary Range USD $58.04 - USD $58.04 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
