Job Description:

Baptist Health is looking for a Physical Therapist to work PRN for our Home Health Agency in Lexington, KY.

Performs initial and ongoing assessments.Establishes and updates the plan of care in concert with the physician.Communicates the patient’s needs/changes in condition to the physician and team members.Accurately documents all care and coordination of care within the specified time according to system and agency policies, regulatory and payor requirements.Incorporates all regulatory and accreditation guidelines in both planning and providing care.Provides clinical supervision for PTA’s. and home health aidesResponsible to meet minimum productivity.Demonstrates competency in physical therapy skills.Participates in performance improvement activities to maximize patient outcomes and the   patient experience.Attends agency meeting as required and completes all mandatory education according to policy.Other duties as assigned.

Minimum Education, Training, and Experience Required

Advanced Degree PTProfessional license PTValid Driver’s LicenseBLS Healthcare ProviderOne year of experience as a Physical TherapistHome Health experience preferred.

Physical Therapist License, BLS, Home Care.
