Summary The Primary Purpose of the Position is: The Physical Therapist Program Coordinator (PTPC) at this level performs higher-level duties which consist of significant scope, complexity (difficulty), and variety, and are performed by the incumbent at least 25% of the time. The PTPCs at this level have experience that demonstrates advanced practice skills and judgment across one or more areas of physical therapy. This position is for the Home-Based Primary Care Program. Responsibilities Clinical Functions: Patient Care Performs patient evaluations: Veterans are scheduled for evaluation considering caseload complexity and acuity of medical condition. Coordinates Veteran schedule with other interdisciplinary team members. Uses research evidence-based examination and evidence-based exercise prescription for more effective patient care. In addition, coordinates therapy services to non-HBPC Veteran's to perform home safety assessments, to identify and recommend adaptive/assistive equipment needs and recommend home modifications as applicable. Treatment Implementation: Evaluation findings are used to develop and implement comprehensive patient treatment plans including objective, measurable goals and timeliness for completion. These goals are developed in collaboration with the patient, family and/or stakeholder input. Demonstrates a full professional understanding of selective therapeutic techniques and modalities related to disability being treated. Serves as a resource and mentors other, less experience therapists. Develops a treatment plan directed toward achieving maximum level of functioning within the home, consulting with other disciplines as indicated and developing the plan and goals in collaboration with the patient and caregiver. Has advance knowledge and ability to independently complete home evaluations specific to needs of diverse patient population. These evaluations may be utilized to assist with HISA grants and specifications. Administrative Program Coordinator Functions: Provides administrative direction and makes decisions related to the HBPC and MFH geriatrics rehab programs. Coordinate and collaborate with HBPC Nursing and social workers for ongoing training and education in assistive devices, durable medical equipment, falls, PT and OT services and referrals Coordinate with program manager orientation schedules of new PT, OT or PTA employees. Assist in providing orientation or needed information to new HBPC STAFF with various disciplines. Grade Determinations:GS-13 Physical Therapist (Program Coordinator): Experience: In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level. Examples of the GS-12 grade level include but are not limited to: independently providing assessment and treatment interventions to inpatients and outpatients; perform examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique personal and environmental factors. PTs interpret findings from examination, diagnostic studies/medical tests and, during evaluation, select appropriate clinical tests and measures, and then integrate findings into the physical therapy plan of care for the full range of patient populations. PTs establish a diagnosis within the scope of physical therapy and identify the appropriate intervention to treat and/or refer to another provider for further consultation when needed. PTs determine physical therapy prognosis by incorporating examination findings with the patient's preferences for meaningful level of function in order to set clinically appropriate treatment goals and optimize outcomes. In addition, the candidate must demonstrate all the following Knowledge Skills and Abilities: (a) Ability to apply advanced knowledge of contemporary physical therapy within specialty area to provide clinical guidance, training or education for internal and external stakeholders. (b) Ability to serve as a consultant within the assigned program area for intra-agency and interagency planning and to provide service coordination to optimize program continuity, efficiency and effectiveness. (c) Knowledge of and skill in management, administration and/or education methodologies. This includes the ability to monitor and track data, utilize available resources effectively, evaluate program quality, and generate reports for local, and/or VISN and/or VACO leadership. (d) Ability to develop and coordinate treatment and/or educational programs within a concentrated field or specialty area, such as, but not limited to, amputee, spinal cord injury, chronic pain, student clinical education/residency programming, polytrauma/traumatic brain injury.
