PHYSICAL THERAPIST (PT) - HOME HEALTH CASE MANAGER
LOCATION: FAR SOUTHEAST SUBURBS WITH FOCUS ON GLENWOOD, HARVEY, LYNWOOD, LANSING, CAL CITY, RIVERDALE AND SOUTH HOLLAND
HOURS: DAYS
This position is eligible for up to a $10,000.00 sign on bonus. You must be working in this same position at time of payout. Current internal team members are not eligible at this time.
Major Responsibilities:
Assuring and Improving Quality of Care: Facilitates the patient and family's right to receive quality cost-effective care following Advocate's MVP of Stewardship.Care and Service Coordination: Practices as an effective member of the patient care team to formulate an integrated approach to care following Advocate's MVP of Partnership.Leadership/Collaboration/Professional Development: Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care following the Advocate MVP of Excellence.Patient Relationship: Develops and maintains a restorative relationship throughout the healthcare continuum, following Advocate's MVP of Compassion.Business Development and Growth: Demonstrates behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment.Patient Education: Demonstrates a commitment to meet the learning needs of patients and their families. COMPENSATION: $86,320.00-$129,376.00 annual base salary
