Key Responsibilities:

Conduct comprehensive physical assessments and review medical histories to evaluate patients. Design individualized treatment plans tailored to each patient's needs and rehabilitation goals. Implement therapeutic exercises and training programs to aid in recovery. Educate patients and their families about condition management, injury prevention, and wellness strategies. Monitor and document patient progress, adjusting treatments as necessary to achieve rehabilitation objectives. Collaborate with multidisciplinary healthcare professionals to ensure holistic and coordinated patient care. Maintain detailed and accurate patient records in compliance with healthcare regulations.

Qualifications:

Master’s degree in Physical Therapy. Minimum of one year of experience as a physical therapist in a home health setting. Current state licensure to practice as a physical therapist. Valid CPR certification. Licensed driver with the ability to travel to various locations as required.
