Physical Therapist (PT) Per Diem

Join our team as a dedicated Physical Therapist (PT) on a per diem basis, where your skills and commitment will significantly impact patients' lives. We are seeking a compassionate and skilled professional to provide therapeutic care aimed at enhancing mobility, reducing pain, and restoring function through tailored treatment plans.

Key Responsibilities:

Review resident information to evaluate its impact on assessments and treatments. Conduct comprehensive functional assessments using standardized procedures. Develop and implement personalized treatment plans with realistic functional goals. Ensure compliance with evaluation, treatment, and documentation standards. Supervise and instruct therapy assistants during treatment procedures. Document treatment progress and revise plans as necessary. Educate residents and caregivers on exercises and procedures for home continuation. Collaborate with nursing staff and families to maximize residents’ functional abilities. Participate in team meetings and quality improvement initiatives. Stay informed of community resources and assist in referral processes post-discharge.

Qualifications:

At least one (1) year of experience as a PT, Home Health experience preferred Valid California PT license required Current CPR certification required  Current California driver’s license, good driving record, and reliable transportation Located in Southern California

Physical Requirements:

Ability to stand for prolonged periods. Capability to push or pull with a force of up to 35 lbs. Ability to lift up to 50 lbs independently and over 50 lbs with assistance. Capacity to stoop, reach, and maintain balance on various surfaces.
