Position Name: **Physical Therapist**
Location: **Sonora** , **Jamestown** , & **Cold Springs, CA** area
Employment Type: Direct-Hire & Permanent
Setting: Home Care and Hospice
**Pay**
- $130,000-$140,000+ (based on experience)
- _Bonus potential:_ based on productivity, every 2 weeks.
**Shift**
- Monday-Friday: 8am-5pm or create your own schedule!
- Can start at 9:30am, 10am, 11am, etc.
- No: on-call, weekends, or holidays.
**Additional Info**

- Supplies kept in the Main Office, go to office and pick up supplies needed
**Qualifications**
_Required:_
- Doctorate or Master's Degree in Physical Therapy.
- CA state PT license.
- 2 years of experience as a PT.
- CPR Certification.
- Driver's License and reliable vehicle with insurance.
_Preferred:_
- Community or Hospice experience preferred. Open to other backgrounds as long as your understand Home Care and Hospice philosophy.
- PT lives in the Modesto, Oakdale, Jamestown, and Sonora, CA area to be able to treat patients in the Sonora, Jamestown, and Cold Springs, CA area.

**Job Summary**
- The Physical Therapist will deliver comprehensive physical therapy services based on personalized care plans.
- Comfortable going to patients' homes and caring and passionate, will take their time with patients and not rush them.
- Patient population _:_ seniors, post-surgical patients, stroke survivors, and individuals with cardiac conditions
_Key responsibilities:_
- Assess patients' therapeutic needs.
- Evaluate physical capabilities (muscle strength, mobility).
- Direct appropriate treatment to enhance comfort and functionality.
- Educate patients and families on therapeutic devices.
- Report changes in patient conditions to physicians.
- Participate in discharge planning.
- Supervise Physical Therapist Assistants or Hospice Aides.
- Provide consultation to caregivers.
- Engage in interdisciplinary collaboration.
- Contribute to quality improvement initiatives within the organization.
