Home Health Physical Therapy Assistant - St. Joseph Home Care Network- Santa Rosa

Schedule: Full Time/ Days
Location: will travel/service around West Santa Rosa and surrounding areas if needed

Job Description:
The physical therapy assistant assists in the provision of physical therapy service provided the service is rendered under the supervision of a physical therapist licensed by the board.  Duties shall include following the physical therapist patient specific established treatment plan with the responsibility to report back to the supervising physical therapist any change in the patient's condition not consistent with planned progress or treatment goals.

Required qualifications:

Completes necessary education to attain Physical Therapy Assistant in state of California. California Physical Therapy Assistant License upon hire. National Provider BLS - American Heart Association within 30 days of hire.

Preferred qualifications:

1 year in Home Health as a Physical Therapy Assistant.
