As a Physical Therapy Assistant in Homecare, you will play a vital role in supporting our clinical team, ensuring our patients receive the best care by:
Competent Services: Competently assisting and contributing to the ongoing patient care plan. Implementing and progressing patients within the established care plan. Reporting observations and patient responses to the supervising therapist and other team members. Recommending discharge assessments in an appropriate and timely manner to the supervising therapist. Consistently following the established care plan. Providing appropriate education to patients and families.Coordination and Compliance: Coordinating timely supervisory visits with the supervising therapist in accordance with the practice act rules and regulations. Demonstrating competent theoretical and technical skills pertinent to patient populations being served under the supervision of a registered physical therapist. Practicing within the Scope of Practice and complying with all policies, laws, licensure, regulations, and standards for care pertaining to the delivery of home care services.Documentation and Monitoring: Providing timely and thorough documentation of the patient record consistent with agency standards. Demonstrating compliance with patient care coordination, including utilization, infection control, improvement in corrective action plans, and required 80% attendance at staff and training meetings.Resource Management: Demonstrating the efficient use of resources by effectively managing time, supplies, and equipment and by meeting established productivity standards.Minimum Qualifications
Valid/current Physical Therapy Assistant License in the state of practice.Current driver's license, insured, reliable transportation, and acceptable driving record.Basic Life Support (BLS) certification for healthcare providers.Ability to access the Internet or a phone line for transferring patient files twice a day.Demonstrated effective communication skills.Basic computer skills.Preferred Qualifications
One (1) year of rehab experience.Physical Requirements:
Carrying, Hearing/Listening, Manual Dexterity, Pulling/Pushing, Speaking, Standing, Squatting/Kneeling.
