Mountainside Residential Care Center – Margaretville, NY

Summary:

We are seeking a caring and personable Advanced Practice Provider (APP), Physician Assistant or Nurse Practitioner, to join our award winning 82 bed enhanced care facility in Margaretville, NY. In this role, the APP will exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services as part of a multidisciplinary medical care team.

Responsibilities:

Conduct thorough physical examinations, assess patient histories, and diagnose common illnesses or conditions among the elderly patient population. Create, implement and regularly review care plans individually created for each patient and tailored to their health needs and goals, in collaboration with the healthcare team and in alignment with facility protocols Maintain accurate and timely electronic health records to document assessments, treatments, and care plans, ensuring compliance with regulatory standards, facility policies, and HIPAA guidelines. Conduct regular rounds to monitor patient’s progress, address any concerns, and provide on-call support during assigned hours for urgent situations Qualifications: Candidates must be board certified as a Physician Assistant or Nurse Practitioner and possess or be eligible to obtain an applicable New York State license. Ideal candidates will have a minimum of 1-year experience, although highly motivated new graduates with interest and abilities in surgery will be considered and are encouraged to apply. Must possess excellent communication skills and demonstrate an ability to work as part of a multidisciplinary team.
