POSITION STANDARDS Unrestricted NH and VT Medical License Required Current DEA license Required BLS Certification Required Board Certified or Board Eligible in Internal Medicine. If Board Eligible, must be actively working toward Board Certification. Subspecialty Board Certified or eligible for subspecialty certification in Geriatric Medicine. Demonstrated competence in providing comprehensive primary care to geriatric adults as delineated on the Clinical Privilege list. Demonstrated competence in providing care to geriatric adults with complex or serious medical problems as delineated on the Clinical Privilege list. POSITION PHYSICAL REQUIREMENTS Anything listed here requires a pre-employment physical by Employee Health to determine if the employee is capable of meeting the requirements. Position Objective Provides continuing and comprehensive primary care to Geriatric patients. Provides care to Geriatric patients with complex or serious medical problems and/or co-morbidities. Provides home care to frail elderly patients who qualify for such care.

 

Performance Expectation Evaluate all assigned geriatric patients. Obtain an appropriate history and physical exam. Order and interpret diagnostic laboratory and imaging studies. Provide appropriate treatment to all assigned geriatric patients. Obtain consultation as clinically indicated and appropriate. When available, provide consultative services for inpatient or outpatient geriatric patients as requested by managing practitioners. Document the history, physical exam, laboratory and imaging studies as well as treatment in the medical record. Obtain appropriate informed consent for all procedures as described in APD Medical Staff Bylaws, Rules and Regulations and Policies. Complete all inpatient and outpatient hospital records within 30 days of day of service. Provide consulting providers with complete and accurate medical records. Communicate in a timely and collegial manner. Communicate with practitioners who request consultation in a timely and collegial manner. Review previously ordered laboratory and imaging studies and determine appropriate course of treatment for patient based on results. Communicate results to patients in a timely manner. Document all “after-hours” communication with patients. Provide call coverage for call group's established adult patients. Provide ER call coverage for unassigned medical patients. Treat all patients and staff with respect. Participate in staff education and clinical Quality Assurance/Performance Improvement activities. Attend clinic staff meetings when requested to do so by the Medical Director or Practice Manager. Abide by CMS Conditions of Participation for Hospitals. Abide by the APD Medical Staff bylaws, Rules and Regulations and policies. Abide by all APD policies.
