Physician - Junior Faculty - FM OB

Job Summary
To provide Junior Faculty advanced training in low-risk and high-risk obstetrics and newborn care to Family Medicine Practitioners who will work in underserved communities, or as faculty.

Essential Functions
GOALS:\r\r1.\tManage simple and complex labors.\r2.\tAttain competency in operative obstetrics, including Cesarean section, instrumented deliveries, tubal ligation, and amniocentesis for assessment of fetal lung maturity.\r3.\tProvide competent prenatal care to pregnancies complicated by diabetes, hypertension, twin gestation, thyroid disease and mental health disorders.\r4.\tStabilize and treat pregnant patients hospitalized for pre-term labor, pre-eclampsia, diabetes Class A, and placental disorders.\r5.\tPerform biophysical profile and limited obstetric ultrasound.\r6.\tCounsel obstetric patients regarding genetic risk and know the appropriate patient and timing to refer for CVS vs. Amniocentesis. \r7.\tResuscitate distressed newborns. \r8.\tProvide ongoing care to the hospitalized newborn.\r9.\tMaintain Family Medicine clinical skills. \r10.\tDevelop and teach competency in obstetrics to Family Medicine residents and family practitioners.\rOBJECTIVES:\r\r1.\tEvaluate obstetric patients and their fetal monitoring strips in triage.\r2.\tProvide labor management, vaginal delivery and instrumented deliveries to obstetric patients.\r3.\tPerform Cesarean sections as primary surgeon. Surgical skill level at the beginning of the fellowship will determine when the fellow is ready to be the primary surgeon. \r4.\tPerform post-partum tubal ligation.\r5.\tPerform urgent/emergent D&C’s on emergency room and post-partum patients.\r6.\tEvaluate and co-manage ectopic pregnancies from the emergency room.\r7.\tSupervise admissions and daily management of antepartum service patients.\r8.\tPerform amniocentesis for fetal lung maturity on the antepartum service.\r9.\tProvide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine clinics for difficult pregnancy.\r10.\tPerform limited obstetric ultrasound at the Family Medicine center, i.e. Biophysical profile, dating, placenta placement, position.\r11.\tPerform newborn resuscitations under the supervision of Neonatal Nurse Practitioner and/or Neonatologist. Resuscitations will include intubations and umbilical vein catheterization.\r12.\tAdmit and manage newborns in step down and/or special care nursery.\r13.\tContinue to see continuity patients 1-4 half-days a week. Supervise resident care of Family Medicine patients’ 1-2 clinics a week. \rROTATION GOALS AND OBJECTIVES (see below for goals and objectives):\r\r6 Months\t\tL&D (BETWEEN OHMC, OAH)\r1 Month\t\tUltrasound\r1 Month\t\tGenetics\r1 Month\t\tNeonatal/ICU\r1 Month\t\tElective\r1 Month\t\tGyn-Onc\r1 Month \t \tPerinatology\r\rI.\tLABOR AND DELIVERY ROTATION (6 Months):\r\rA.\tGOALS:\r\r1.\tManage simple and complex labors.\r2.\tAttain competency for operative obstetrics, including Cesarean section, instrumental deliveries and tubal ligation.\r3.\tProvide post-operative care for such patients.\r4.\tDevelop and maintain competency in teaching obstetrics to Family Medicine residents.\r5.\tResuscitate distressed newborns.\r6.\tProvide ongoing are to the hospitalized newborn.\r\rB.\t OBJECTIVES: \r1.\tEvaluate obstetric patients and their fetal monitoring strips in triage.\r2.\tProved labor management, vaginal and instrumented deliveries.\r3.\tPerform Cesarean sections as primary surgeon with obstetrical physicians.\r4.\tTeach 1st year residents the OB service basic obstetrics, cervical exams, labor management, Pitocin augmentation, IUPC, FECG, vaginal delivery and post-partum care.\r5.\tPerform post-partum tubal ligations.\r6.\tPerform neonatal circumcisions.\r7.\tPerform urgent/emergent D & C on ER and post-partum patients.\r8.\tEvaluate and co-manage ectopic pregnancies.\r9.\tPerform newborn resuscitation under the supervision of the Neonatologist (or II.\tULTRASOUND ROTATION (1 Month):\r\rA.\tGOALS:\r\r1.\tPerform ultrasound during pregnancy. \r2.\tBecome competent in performing biophysical profiles, AFI, EFW, fetal position, placenta placement and EDC.\r3.\tUse ultrasound skills in the Family Medicine office, L & D and Antepartum ward.\r4.\tTeach ultrasound skills to Family Medicine residents and faculty. \r\rB.\tOBJECTIVES:\r\r1.\tPerform ultrasound on obstetric patients when needed.\r2.\tSupervise Family Medicine residents in performing ultrasounds.\r3.\tPerform amniocentesis for fetal lung maturity when required. \r4.\tProvide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine residents. Units may be in consultation with perinatologist.\rIII.\t GENETICS ROTATION (1Month):\r\rA.\tGOALS:\r\r1.\tAttain competency in identifying which patients need formal consultation with MFM and/or Genetics.\r2.\tCounsel patients appropriately on CVS vs. amniocentesis, i.e, which patients are better suited to which procedure.\r3.\tAppropriately counsel patients in the office setting concerning quad screening and first trimester screening. Accurately counsel patients on how to proceed with abnormal results.\r\rB.\tOBJECTIVES:\r\r1.\tEvaluate obstetric patients through consult and discussion.\r2.\tTo be able to identify which patients need formal consultation. \r3.\tTo be able to discuss each procedure, pros, cons and percentage of risks.\rIV.\tNEONATAL ICU ROTATION (1 Month):\r\rA.\tGOALS:\r\r1.\tResuscitate distressed newborns.\r2.\tProvide ongoing care to the hospitalized newborn. \r\r\r\r\rB.\tOBJECTIVES:\r\r1.\tPerform newborn resuscitations under the supervision of the Neonatologist, including intubations and umbilical vein catheterizations.\r2.\tContinue management of newborns in special care nursery.\r3.\tPerform complete resuscitation and newborn care as part of the neonatal resuscitation team.\rV.\tELECTIVE ROTATION (1 Month):\r\rA.\tGOALS:\r\r1.\tTo attain increased comfort and knowledge in OB and/or Family Medicine.\r2.\tTo obtain additional training suited to individual clinical practice.\r\rB.\tOBJECTIVES:\r\r1.\tAttain competence in specific procedural or cognitive skills, which may include increased competence in colposcopy/LEEP, ultrasound, Family Medicine clinic, more advanced surgery.\r2.\tTo have continuity clinics continue through the month.\rVI.\tGYNECOLOGY/ONCLOGY ROTATION (1 Month):\r\rA.\tGOALS:\r\r1.\tDevelop increased skill at recognizing abnormal patterns with colposcopy. Understand and be knowledgeable on the different cervical conization procedures.\r2.\tKnow the indications for cone and be able to counsel patients.\r3.\tCompetently perform LEEP procedure.\r4.\tRecognize and manage complications from LEEP. \r5.\tKnow indications for vulvar biopsy and treatment.\r6.\tDetermine when and if endometrial sampling is necessary and be competent in the procedure (D&C vs. Endometrial sampling).\r\rB.\tOBJECTIVES:\r\r1.\tTo attain competence in specific procedures.\r2.\tSupervise and teach fellow Family Medicine residents indications and procedures.\ra.\tI.e.: LEEP, endometrial biopsy, vulvar biopsy, D & C and Hysteroscopy, as applicable.\r3.\tScrub in as 2nd or 3rd assistant in major Gyn-Onc surgery. To become familiar with the procedures.\ra.\tI.e.: Length of procedure, estimate blood loss, risks involved and post-op care.\r4.\tCounsel patients about procedure and follow-up care, as applicable.\r\rVII.\tPERINATOLOGY ROTATION (1 Month):\r\rA.\tGOALS:\r\r1.\tProvide competent prenatal care to pregnancies complicated by diabetes, hypertension, twin gestation, thyroid disease, cardiac disease, renal disease and mental health disorders\r2.\tStabilize and treat pregnant patients hospitalized for pre-term labor, pre-eclampsia diabetes and placental disorders.\r3.\tPerform biophysical profile and limited obstetric ultrasound.\r4.\tCounsel patients regarding genetic risks.\r\rB.\tOBJECTIVES:\r\r1.\tSupervise Family Medicine OB admissions and daily management.\r2.\tPerform amniocentesis for fetal lung maturity.\r3.\tProvide initial consultation and ongoing prenatal care to patients. Focus will be on patients referred from Family Medicine residents.\r4.\tPerform limited obstetric ultrasound and biophysical profiles.\r5.\tCo-counsel patients referred to genetics counselors.\r6.\tSee patients with perinatologist in MFM clinic.\rOTHER GOALS:\r\rI.\tCONTINUITY CLINIC:\r\r1.\tMaintain knowledge and skills as Family Practitioner.\r2.\tBuild a practice suitable for billing.\r3.\tPerform prenatal care for their private patients and be responsible for their care in L & D.\r4.\tSee patients on the same schedule as attending physicians.\r\rII.\tPRECEPTING:\r\r1.\tMaintain knowledge and skills as Family Practitioner by working with residents.\r2.\tTo be able to teach fellow Family Medicine resident procedures and indications, incorporating their new knowledge in Ob and Gyn procedures along with Procedures normally don in Family Medicine office.\r\rIII.\tTEACHING:\r\r1.\tGive two OB and Gyn lectures at the Wednesday morning Family Medicine lectures.\r2.\tGive some of the lectures and presentations for the medical students and OB/Gyn residents while on the L&D rotation.\r3.\tPresent interesting OB/Gyn cases at the Family Medicine case conference on the last Thursday of each month.\r\rREADING RESPONSIBILITIES:\r\r1.\tGabbe-Pertinent reading to be outlined\r2.\tWilliam- Pertinent reading to be outlined\r3.\tDroegenmueller- Pertinent reading to be outlined\r4.\tACOG Compendium\r
