Through this community of healers, scientists, and scholars, Rutgers is equipped as never before to transform lives. Posting Summary Rutgers, The State University of New Jersey, is seeking a per diem Physician Specialist for the department of Medicine at the Robert Wood Johnson Medical School.

In this position, the general internal medicine doctor will manage a panel of primary care patients at Eric B. Chandler Health Center. This physician will have a continuity outpatient practice 1-2 half-days per week where they will oversee the care of 11-12 patients per ½ day session while precepting Rutgers RWJMS internal medicine residents. The physician will care for the ongoing medical needs of an underserved population as well as the acute care needs of post RWJUH hospital discharges. The physician will collaborate with subspecialists at the RWJMS practice as well as with the on-site services provided by EBCHC such as diabetes education, nutrition, podiatry and dentistry. They will be responsible for checking and responding to their in-basket messages and communicating with nursing staff.

Among the key duties of this position are the following: Manages the chronic and urgent outpatient needs at of 11-12 patients per half day session at EBCHC. Teaches and precepts Rutgers RWJMS internal medicine residents per session. Collaborates with consultants and subspecialists at RWJMG outpatient practice. Manages the transition of care from RWJUH to the EBCHC ambulatory setting post hospitalization. Manages in-basket messages from patients, nursing staff and pharmacy, with the help of the IM residents. Engages in teamwork with nursing staff, social workers, community health workers, pharmacy, and subspecialists to provide optimal care to the underserved patients at EBCHC.
