As a Physiotherapist, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do?

The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for ONTARIO HEALTH ATHOME Champlain Patients in a variety of settings (in homes and retirement homes)

 

Primary Responsibilities:

Provides physiotherapy services to ONTARIO HEALTH ATHOME Champlain patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient’s condition.Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.Ensures that clinical records are maintained in accordance with ONTARIO HEALTH ATHOME Champlain Policies and Procedures and the College of Physiotherapists of Ontario.Schedules patient visits according to Physiotherapy priorities.Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals.Acts as an education and resource person for other health care professionals.Participates in committees as approved.Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy.Maintains supplies and recommends equipment for department use within budget limitations.Interprets the role and objectives of the ONTARIO HEALTH ATHOME Champlain Physiotherapy Program in the community.

What must you have?

Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of OntarioRelated professional experience which would normally be attained over a period of two years in a Physiotherapy roleRelated experience in a similar community-based setting is an assetCurrent CPR-C certificateWe have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, etc.)Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capabilityStrong ability to manage difficult conversations/situations in a healthy and effective manner
Demonstrated ability to work effectively in both a team environment and independentlyEffective communication skills to establish and maintain a wide range of contacts with professionals and organizations within the communityDemonstrated strong customer service skillsEffective listening and strong facilitation skills are necessary to work with patients and their families/caregivers/SDMs in order to ensure best possible service is provided to the patientAbility to deliver information effectively in a variety of settings including one-on-one, team meetings, and to assist in delivering presentations/information regarding the ONTARIO HEALTH ATHOME Champlain Physiotherapy programA valid driver’s license and access to a vehicle is requiredFluency in English and French is required.Vulnerable Sector Check completed within the last six months

 

Hours of Work

Monday to Friday – 8:30am to 4:30pm (35hrs/week)
