Wage: starts at $22+/hour, depending on experience

Location: NW Metro (coverage area: Ramsey, Dayton, Rogers, Maple Grove, Plymouth, Medina, Delano, Montrose, Buffalo, Maple Lake, Monticello, Big Lake, Elk River, Nowthen & surrounding). Plus mileage reimbursement of .67 cents per mile.

Schedule: minimum of 10 hours/week up to 40 hours/week (this position has an option of being part-time or full-time)

The Telehealth Coordinator - Population Health serves a generalist role and will be responsible for the clinical, clerical and administrative duties related to telehealth services at the Community Population Health level. This individual serves to coordinate telehealth services for the members at the distant site and supports the provision of care by other health care system personnel. The wide range of tele-health services requires that the clinician have competencies and skills in a variety of specialty areas related to the tele-health appointment.

 

This team member will also support member care and be responsible for delivering care in the primary care, community-based setting. Important roles may include performing preparing and “rooming” members, coordination of clinic supplies, and clerical support.

 

Position Functions and Responsibilities:

Has the knowledge and skills necessary for carrying out practical care assignments. Serves as a member of the Community Based team and provides care to the community panel of members. Completes assigned duties and responsibilities which involve performing nonstandard and specialized personal care activities and requires a broad work experience and skills. Completed work requires only a general review by a registered nurse or advanced practice provider for technical soundness, appropriateness and conformity with policy and assignments. Responsible for the member site facilitation of the clinical, business and technical aspects for telehealth applications using video-conferencing technologies. Demonstrates knowledge and skills to perform personal care and assistance activities related to the tele-health visit, primary care visit, or nursing telephone care. Performs administrative tasks related to patient scheduling, supplies, collection of pertinent data for regular and one-time reports. Accurately reports and records all pertinent screens, medical histories, assists with medication reconciliation, clinical reminders, vital health information in the medical record. Communicates significant findings to the provider or Life Manager. Prepares the member and telehealth room with the appropriate equipment and supplies. Readies the equipment for exams and establishes the video conferencing connection with the provider’s location. Shares responsibility for ensuring that the medical equipment is functional, properly cleaned, and safe. Reports any evidence of malfunctioning or problems to IT department or clinical supplies. Learns, maintains and demonstrates competencies necessary for assisting with all procedures, physical exams and equipment necessary to carry out all duties related to telehealth and primary care. Accurately documents these activities and performs them according to policy and procedures. Communicates technical issues and problems to IT HELP desk.  Orients the member to the tele-health process and equipment. Stays in the room if this is part of the clinical protocol or as the member/IDT requests. Assists with the tele-health visit during the diagnostic interview, exam, procedure and follow up plans. Coordinates and supports Clinic Back Office telemedicine coordinators/schedulers regarding provider and member scheduling changes, appointments, and logistical issues. Prepares the member for discharge from telehealth services ensuring they understand the next step in their care. Accurately assists with workload capture basic for the visit's completion. Assists with the coordination of telehealth handoff to the primary care provider. Complies with all safety and security regulations related to member care. Use appropriate personal protective equipment when required. Performs other clinical and administrative duties as assigned specific to the unique workload and staffing demands of the Community Based Population Health group. Must be able to work independently and remain calm in a fast-paced environment. Customer Service: Listens to the members to correctly identify and assist in resolving member-related issues. Maintains confidentiality and privacy of member related verbal, electronic and printed communication. Provides compassionate and considerate care to members and their families. Functions as a team level patient advocate by identifying resources for members and families. Serves as a liaison between the providers, members and other clinical personnel. Communication: Establishes effective and timely methods of communicating pertinent member findings with other team members and consultants. Attends and participates in team meetings, telehealth coordination meetings to enhance communication and performance improvement activities. Assists with new staff clinic orientation through shared knowledge and clinical expertise. Establishes constructive relationships with patients and team members using methods that enhance patient satisfaction.

 

Experience and Education:

At least 1 year of experience working with complex geriatric clients in community, congregate, and/or home-based environment. Graduate of certified nursing assistant or vocational nursing program preferred, but not required. Experience participating in clinic level, service line and educational and performance improvement activities and committees.
