The Population Health Analyst will support the Manager of Value Based Care Operations with population health initiatives. This will include managing data, aggregating reports, and analyzing and presenting findings for Saint Luke's Physician Group practices. Support the management of regulatory reporting for Medicare value based programs for all primary care practices. Review Epic charts for documentation, submit charts to payors in accordance with plan program requirements, and identify care gap opportunities. Become a subject matter expert in quality measure closures, assist in educating providers and reviewing charts on documentation requirements, and identify areas for improved documentation.
Key Responsibilities:
Work closely with Manager of Value Based Care Operations in the strategic approach to value-based care performance.Support other analytics resources to define requirements, design and validate value based care performance.Engage staff and stakeholders in continuous improvement initiatives to enhance program outcomes.Support compliance with regulatory requirements and maintain accurate program documentation.Serve as a liaison between the organization and external partners, including payers and regulatory bodies, to facilitate effective communication and collaboration.Become intimately familiar with all payer portalsUnderstand each payer’s attribution logic and ensure our provider and patient rosters are accurate.Knowledge of reporting availability from individual payer partnersUnderstand upload processes for HIPPA compliant data sharing with payer partners.Translate EMR and Payer gaps in care for directing administrative closures.Analyze data to inform decisions regarding resource allocation and program development.Job RequirementsApplicable Experience:
