Job Objectives
The Population Health Clinical Navigator is part of the multidisciplinary Care Management team. Responsibilities include outreach to patients to educate on appropriate utilization of health care services. Requires collaboration with GRIPA and RRH care managers, physicians, other entities (insurance companies) and all other health care professionals at various sites of service.

Required Experience:
