· Works in collaboration with the UVA Health Population Health case managers, specialists, clinicians, and team members to identify clients at intake who are in need of Community Health worker services.
· Complete patient screening to determine eligibility for Community Health worker services.
· Collaborate and consult with staff to identify current clients who are in need of Community Health worker services.
· Provide SDOH screening to each client enrolled in the program and connect clients with resources to remove SDOH barriers to healthcare.
· Respond within 24 hours to requests for Community Health Worker services.
· Provide feedback to the patient’s provider care team and others involved in the patient’s care or support.
· Assist clients with establishing a primary medical home, transportation to health-related appointments, and assist with follow-up and scheduling of additional appointments.
· Explaining the basics of wellness education, chronic health, and self-care.
· Documentation into each client’s EMR using Epic.
· Assist clients with goal setting, financial assistance, Medicare/Medicaid applications, or any other forms and applications needed.
· Connecting people with services; assisting with appointments and transportation.
· Meet with clients at their homes, clinic appointments, or community events.
· Acting as a liaison between clients and care providers and the navigation of a complex healthcare system.
· Being a \"voice of experience\" and facilitating support and potentially education groups.
· Other duties as assigned.
Position Compensation Range: $17.31 - $26.83 HourlyMINIMUM REQUIREMENTS
Education: High school diploma or equivalent
Experience: None
Licensure: Certification as a Community Health Worker (CCHW) within 18 months of employment.
PHYSICAL DEMANDS
This is primarily a sedentary job involving extensive use of desktop computers. The job does require traveling some distance to attend meetings, see program enrollees, and manage program duties as assigned.
