Post Acute Network Supervisor
Overview M Health Fairview has an immediate opening for a Post Acute Network Supervisor. This is a 1.0 FTE, salaried/exempt (80 hours per two-week pay period), benefits-eligible opening. Responsibilities Job Description Job Summary: This position will serve as a liaison between the contracted post-acute network and the various entities of Fairview (hospitals, clinics, and M Health Fairview Geriatrics). This position will be responsible for leading initiatives from planning through implementation stages, interacting and influencing various staff across the system as well as with outside providers which include skilled nursing and transitional care facilities and other key stakeholders to advance key initiatives for the Post-Acute Network. Job Expectations: Serves as the liaison between the Fairview Partners network TCUs/SNFs and the hospital care management staff. Optimizes relationships among the post-acute network. Provides monthly check-ins with certain identified facilities; potentially forming an formal/informal tighter sub-network of the Fairview Partners facilities. Keeps Fairview Stakeholders continually informed on what is valuable for us to know. Holds regular meetings with the hospital care management leaders. Coordinates clinical training among the TCUs/SNFs so they can accept increasingly complexity of patients. Bridges Fairview nursing education to facilities as necessary. Utilizes feedback from the M Health Fairview surveys that relate specifically to the facilities to share feedback with the facilities. Shares discharge trends/open beds. Utilizes reporting to understand acceptance rates and trends going on with the post-acute partners. Develops metrics to track in partnership with the facilities and communicate metric results monthly. Assures facilities are on Epic and educates as appropriate. Provides Fairview Partners training to new leadership staff at partner SNFs/ALs. Serves in a lead clinical role to facilitate the monthly SNF interdisciplinary care team rounds. Oversees the Fairview Partners grant program. Oversees the Education and Enrollment (E&E) functions including: Supervises E & E Specialists, ensuring they maintain their insurance sales licensures and comply with health plan and state enrollment regulations. Attends annual health plan compliance training to fulfill health plan and state enrollment regulations. Is available to assist E & E Specialists with any provider and enrollment issues they may encounter. Attends facility enrollment training sessions as necessary. Assures E & E territories are covered appropriately and ensures a plan is in place for E & E coverage. Conducts weekly Daily Engagement System meetings with E & E Team. Maintains policies and procedures related to enrollment and marketing. Provide support on the marketing functions of the Fairview Partners program including: Updating advertisements Marketing/giveaways Collateral materials PowerPoint presentations Transitional Care Planning SharePoint site - keep updated with patient and staff materials Assists with entering and exiting network facilities: Tours new facilities and brings recommendations to the Fairview Partners Management Team. Meets with new facility staff and trains on the Fairview Partners program. Supports the E & Es during large member enrollments and exits during facility transitions. Qualifications Required Education Degree in nursing Experience 5-7 years in a clinical setting. 5-7 years working with project management/project administration and senior leadership.
